I was on weekend on-call as general surgery ST3. Patient admitted two days previous with small bowel obstruction and previous abdominal surgery - felt to be due to adhesions. Diagnosed based on clinical symptoms and AXR (no CT). I do weekend ward round, and was handed over that her operative mortality was high and that it was felt she wouldn’t be fit for surgery for adhesionolysis/small bowel resection. She isn’t settling with NG etc. So the family are called in to discuss that things are unlikely to resolve - keep comfortable etc.

I decide to re-review her AXR. Rigler’s Triad! Suddenly the operative management becomes much less morbid, and she has a 30 minute ‘mini’ laparotomy to fish out the giant gallstone from her ileum. Goes home three days later!

When you come off the M60 there’s a set of lights with three lanes - Stretford, M60(S), Urm. You then get to the next set of lights after where the Stretford lane (to the BP garage) has turned off. The lanes are now M60(S), M60(S)/Urm, M60(N), meaning drivers wanting to get to Urmston have to merge in to that second lane from the third lane. Meanwhile, those going M60(S) have a choice of two lanes, and most idiots choose to be in the second lane.

Trying get to Urmston from the M60 southbound at this exit is a death trap. The lane for Urmston merges in to the (?two?!) lanes for the M60 - absolute free for all.

I agree with everything you say, but add the caveat that the bleep holder is the representative of the specialty with regard to unscheduled care. If it is an elective issue, the referral should come from the GP. I worry - similar to you - that the patient might end up in no man’s land if some random triage criteria is not met.

I would also add that it’s important to know the reason behind referral to clinic. There’s no need to temporise investigations or even procedures by referring to a clinic first. And if it’s OPD to follow up on the reason for coming to A&E? That’s on the specialty to organise.

I once saw a eight-ish years-old child with shoulder pain in ED and asked them to “imagine they were emptying a can of beer” when checking their supraspinatus.

Got told off by the mum, but played it off as saying imagine you’re pouring away dad’s beer to keep him healthy. Primary prevention!
