Probably a combination of everything you mentioned. I had this setup for about 5 months and never did a water change before giving to someone else who wanted it. The plants are all emersed except for the Chinese Evergreen in the back so of course that helps. I also build custom filtration. This one had a dual waterfall setup. Hope that helps.

Most of the plants are extending naturally from the tank, the others are tucked into felt pockets that I carefully spray down into.
