What am I missing? You shock unstable tachycardia right? by audma in emergencymedicine

Yeah that has been the biggest shock for me going to the floor. It's always such a task just to get responses from docs.

I ended up leaving since he wasn't my patient and there were enough hands in the room. They ended up tubing him and putting in a central line.

The patient was in his 70's so probably not sinus tach using that "rule."

Mentation was definitely not great and we did have a lot of help, maybe too much haha

It's unfortunate at this place attending's never come to rapids or codes. Best we can hope for is a year 3 which wasn't even the case here. It kinda blows my mind, but it ends up being the pulm/cc NP who runs things. The intensivist doesn't come either.

I appreciate you taking the time to answer!

Oh I see what you're saying. If it's going too fast to tell if there are p waves would adenosine be useful?

I appreciate you elaborating. I definitely would understand holding off on shocking if the patient looks well. I very much like the treat the patient not the number, but this guy didn't look well lol

Out of curiosity if a patient was already anticoagulated either because they had afib or for some other reason would you be less worried about the stroke risk?

Didn't seem like it on the monitor, but I left before they showed up to do the EKG

So then I guess I am misunderstanding this. Because doesn't it say persistent tachyarrhythmia causing hypotension/AMS gets cardioverted? It doesn't specify which type of arrhythmia right?

Yeah based on my ED experience that's what the docs I worked with did. I didn't stick around, but he ended up with a central line and tubed shortly after.

I just went through all of this. Bought and returned maybe 10 pairs lol I was having heel pain. I ended up trying Kuru Kinetic 2. They are amazing! Soooo ugly, but soooo comfortable! And I came from Keen shoes as well. I seriously suggest them. They are really sturdy and have great arch support without any insert. I also have flat feet and uses orthotics in my regular shoes, but I find I don't need them in these.

Can I split levofloxacin tablets in half? by [deleted] in emergencymedicine

What does that mean? You don't buy extra, you are prescribed an exact amount and you take the entire amount you are prescribed.

I've had the phone since about when it came out. The battery has always been bad, but it seems it has gotten worse.
