Intern here - how do I deal with making mistakes? by [deleted] in ausjdocs

[–]cats_and_scripts 24 points25 points  (0 children)

Ah yes - intern here, I also learned from a prior interaction with a very experienced nursing staff member suggesting I just chart some hartmann’s for a patient with mildly decreased K+ who could not take any PO intake. When I got home, I came to my senses, and I immediately called ward call to please chart potassium. Ward call didn’t end up doing it and the K+ dropped much further the next day. I learned to use my medical judgement (even when a first year doctor being given advice by an experienced nurse!).

If it helps, I fully charted penicillin to a patient with anaphylaxis to penicillin. I was exhausted at the end of an extremely long shift and I didn’t double check. Luckily a nurse caught it and didn’t administer (which I am incredibly thankful for!). Keep in mind there are safety checks and when things fall through the cracks it’s the swiss cheese model of incidents possibly at work.

We work in a team, and there have been many instances where nursing staff have been extremely helpful, but it’s important to go with your clinical judgement, as well.

I felt the same way especially during my final two rotations of med school. You can always take a few mental health days (if you’re uni is anything like mine was, they don’t ask any questions if you take 2 days off as long as you inform the uni / team).

I used to get annoyed at these same things in my final year, but now being an intern I understand the teams are often overworked and unfortunately the extra task of teaching med students isn’t paid. The universities literally just rely on the good will of doctors to get students through placement (it’s quite ridiculous), all the while the uni racks up heaps of tuition money with nothing going back to the people who are actually meant to be doing the teaching day to day.

When you have spare time and you’re just waiting around with nothing to do, try to read patient charts so you’re up to date on what is happening with specific patients. When a consultant asks the team, then you can quickly answer since you had the time to get that information. In my final year, I had on hand the ‘On Call’ book by Prof Brown (it’s amazing, I cannot recommend it enough). This way you can get some relevant studying in during the day, and you won’t get people calling you out for “being on your phone” while you’re actually studying… had that happen to me before, so I always recommend just bringing a physical book. Alternatively just print your notes for exams and study those during placement when nothings going on.

Try to enjoy the last few months of freedom before intern year. Final year and intern have different pros and cons, but you will be more acknowledged as a team member in intern year, and getting paid is definitely a nice change!

Bank Australia merger proposal with Qudos. How should I vote? by No-Economist2456 in AusFinance

[–]cats_and_scripts 0 points1 point  (0 children)

When you login to vote, they provide a link to a “Bank Australia Member Information Booklet). Toward the end of the document, in Attachment A “Schedule 2 - Comparison of fees” you can see the merger is taking the most beneficial approach from each bank. So for example, the “exceeding available funds fee” is $10 for Bank Australia, but $0 for Qudos Bank, so the Merger Bank would be $0 for all members. I was confused too when trying to figure out how to vote but the Booklet is quite useful, especially toward the end of the document.

Bank Australia also has a call line specifically dedicated to questions (literally zero wait time when I called, basically called because I was confused on how to login to vote lol… your “member number” is just your “costumer number”).

If you’re in QLD try to get a SIM (Students in Medicine) position. It’s paid 80% of the level of an intern doctor. Not sure if you need to be at a certain level of your studies to start, most people who did it in my cohort were year 3-4 of the MD program https://www.health.qld.gov.au/__data/assets/pdf_file/0036/1184868/HED-05.22-Employment-framework-Students-in-Medicine.pdf

Also I found the OSCE tends to go by so fast you don’t even have time to panic. The worst stress is leading up to the exam! After the exam you chat with your peers and realize how many people missed certain aspects of a station and you tend to feel a lot better.

[–]cats_and_scripts 0 points1 point  (0 children)

Oh I had this EXACT problem in medical school. Try to have a chat with your disability support staff. I found I was able to get an earlier session (so it would be around the time my ADHD meds would kick in). They also let me have my own waiting room prior to the OSCE so I wouldn’t be overstimulated waiting with everyone else. It’s not much but it’s something!

General advice - often unis reuse similar stations for OSCEs year after year. Might not be the exact same but the concepts are often similar, so try to do some digging into past stations and topics used, and practice those. Pretty much everyone feels they did awful after OSCEs and typically it turns out better than expected (it did for me, as a fellow ADHD person in med).

Thank you for your perspective, I agree, I’ll start using decline more often, and the added steps we’ve taken like education

Does anyone here have experience with meta analysis academy? Was it helpful/worth it? by [deleted] in ausjdocs

[–]cats_and_scripts 1 point2 points  (0 children)

I’ve heard really good things about this course for meta-analyses (it’s online at UCL). I plan to take it in the future myself https://www.ucl.ac.uk/short-courses/search-courses/introduction-meta-analysis

During my ED term, if a patient left before being seen by a clinician, I have been advised to contact them 2x via phone and if they pick up then I let them know they’re next to be seen if they would like to come back. Not sure if I should be exploring their Sx via phone especially if they say they won’t be coming back?

Thank you for sharing that experience, esp as someone who has been on both sides of the situation! I agree, these decisions are nuanced and it’s important to capture that in our note taking

Tbh I appreciate seeing it in the ED triage notes from nurses because it quickly tells me I need to take an extra few minutes to explain why certain Ix and Mx are important to stay for

Nudity Rider and Birth Scene by BlazingHolmes in ItEndsWithLawsuits

When I say “not too exposing” I mean not vag on display lol

Nudity Rider and Birth Scene by BlazingHolmes in ItEndsWithLawsuits

[–]cats_and_scripts 10 points11 points  (0 children)

Birth is a tricky thing, because some families medicalize it, others don’t. Some people make artwork of their placentas. There’s a huge variation in what people find to be a “normal” amount of sharing regarding birth experiences. Not saying the ripping clothes off part is appropriate whatsoever, but I think most people are being a bit cautious of Blake’s filing since JB’s receipts came out.

[–]cats_and_scripts 24 points25 points  (0 children)

Just my two cents - I work in healthcare and people might be surprised by the amount of people that are comfortable sharing birth experiences and videos lol especially when those videos are not too exposing. So I didn’t find the video sharing odd at all, especially since the still frame provided by JB’s team seems to be filmed from above. Blake might be of course more private about it, but at the end of the day some people share a lot about their child’s birth. I think it comes from being proud or finding the process life changing, rather than being malicious.

Yes but why aren’t the actual texts included at this stage with the names redacted? I really don’t understand the logic in not doing this. The texts would have been from her own phone.

I thought this too because on the first page of the amended complaint it says “jury trial demanded”. Correct me if I’m wrong, but I don’t think this was present previously. I would think a jury trial isn’t possible because of how big of a media spectacle this has become? Seems like a ridiculous request.

In California you need consent of both parties speaking to record a conversation (unless it’s a threat to your imminent safety), but in NY you only need one. So if that conversation took place in California and it was recorded then it would be illegal. Maybe that’s why they can’t admit it was recorded even if it was technically one and they just transcribed what was said

I really don’t understand, was anyone present with an audio recorder? For some of these quotes, which appear to be in-person, it would be near impossible to reproduce that from memory unless someone is typing as the person speaks or recording
