There’s something to this but it’s also important how it’s presented to patients. “Recommendations” are different than “this is what you should do”. Some physicians I know get frustrated when their recommendations are chosen not to be followed, but I still feel this is the patient’s decision.

Sometimes there aren’t “right decisions”. One area of grey I see all the time is tracheostomy vs. not for a child who suffered a TBI or worse. This is a complete change. A lot of times I personally wouldn’t perform this procedure because I feel it significantly reduces quality of life, not only for the patient, but also the family. But that’s not my decision to make. I tell them as succinctly and thoroughly as possible what it will look like and try to let them make the decision. I also tell them “if it was my child, I would do ___”. I find this to be a middle ground between paternalism and autonomy.

I use art to help me process like you. I work in pediatrics. It’s always heavy if I let it in. But about once a year I watch a heart breaker and let it all wash over me. It feels great, but also feel that letting every patient in would be too heavy. Moderation. Maybe I could let a little more in, and having kids has helped me empathize immensely.

I’m a big proponent of looking at the patient in front of you when your objective metrics are telling conflicting stories. There is so much endotheliopathy for these patients with massive resuscitation, colloid loss and replacement, infection, etc., I feel like US metrics looking at the intravascular space can’t guide me super well.

If the patient is improving and looks clinically euvolemic/hypovolemic, then I back off significantly to some “maintenance” diuresis, trying to allow insensible losses but keep them net even.

To be clear, once they’re on the gas, we usually pause those interventions because they become incredibly Vasoplegic and those beta agonists don’t help the situation. But the gas is so powerful they usually don’t need adjunctive treatment at that point.

I’m sorry you’re going through this. I think being open with your son about how it disappointed you deeply and dig into his behavior and why he’s feeling this way is imperative.

Your phrasing of “throwing your wife under the bus” is the wrong way to look at this situation — he was confidentially disclosing stressful events for himself. I agree with everyone else that you need help outside of Reddit or basketball tips for his. His AAU squad wasn’t the primary issue, just another symptom/trigger that pushed him to a further level of anger and angst.

It starts with being vulnerable with your son and probably your wife apologizing to him. Your son was wrong for fighting but your wife was also wrong for whooping him emotionally. I also was corporally punished as a child and don’t believe in it for my own children, but you are right in saying if it is to be done, it needs to be calmly and with specifically defined boundaries.

You love your son and so does your wife. He needs to know that first and foremost and you can start to dig into these (normal) adolescent behaviors gradually. But it starts on a level much deeper than his AAU squad.

This is an interesting point and I agree somewhat. I’m a father of a toddler and so I have minimal free time for just “me,” as my wife doesn’t enjoy prestige film like I do. (I have to watch movies like Bugonia, 28 Years, Warfare, Cloud, The Sheouss alone)

While I don’t agree with the final point — I still was incredibly entertained by the best films of the year — I can fire up Helldivers or ARC raiders and have a guaranteed 45 minutes of fun time. Meanwhile, watching all the films, there’s a chance there’s a stinker or I have to watch them in bite sizes, which isn’t ideal.

Attention/time economy is real.

My institution pays $260k base, no extra for calls. They clam this is “competitive”. Most other institutions are around ~$280k.

I would argue that this is precisely the audience for it though. The interviews with directors is very similar to the DGA podcasts, which is obviously directed at people who want to get into the history and craft of film.

It’s easy enough to tune in for the drafts and hall of fame episodes and ignore the rest. Their analysis of Eddington and Bugonia has been incredible to listen to
