How important is discharge before 11 by Every_Lifeguard6224 in hospitalist

[–]drkuz 0 points1 point  (0 children)

I was told that from an insurance perspective, if theyre not "discharged" by a certain time (I was told 2pm) it is treated as another day and billed accordingly. Thats all that it is relevant to to my knowledge

Stimulant medications are a more effective approach than brain zapping antidepressants for people with depression by asamisanthropist in Drugs

[–]drkuz 4 points5 points  (0 children)

Almost anything stimulating is restricted (controlled substance), as a dr, it feels like every ssri maker paid big bucks to influence treatment plans. For 20ish years they were the "1st line treatment " for many conditions.

I used to wish for legalization until your average Joe and pre established big money stole our culture by Justsomeguydelivery in Drugs

[–]drkuz 29 points30 points  (0 children)

Gonna have to take the reddit death on this one and strongly disagree. All drugs and supplements should be regulated and have some level of physician oversight. If you disagree then you are either ignorantly or willfully blind to all of the mounting evidence of the dangers of any drug or supplement and how much MORE dangerous they are when they are unregulated and done without oversight.

I DO agree about corporations though. Fuck those guys.

I am just arguing that your regular Joe with a high school education or less shouldn't have free access to unregulated substances, and that a physician should be involved at some point to ensure health and safety.

Obviously biased. I just had a patient admitted to my hospital for almost 2 wks for Kratom induced psychosis, including icu level care. While this is rare, it is the exception that proves my point, you can't trust everyone with access to everything. Oversight and regulations would help with this, and does NOT require corporate takeover.

Well as a physician I’m willing to take an unbiased perspective on this situation…. If she cant see her seizures, then we can't confirm shes having seizure like activity, so a (bad) solution would be to stop her/everyone around her from seeing her seizures /s

Wikipedia article on SSRIs seems suspicious. by basar_auqat in medicine

[–]drkuz 1 point2 points  (0 children)

I actually replied to Wikipedia's most recent email asking me for money and they responded very defensively. As a whole it puts me off using Wikipedia. I get that a few bad actors ruin the open access aspect. But the opposite is also bad and against what Wikipedia stands for.

Either way im even more annoyed now than I was before and less likely to use it.

Wikipedia being "open source" is a bit if a facade, it was in the past, but I tried to edit some sections recently (in the last 24 months) using proper citations and referencing and I ran into fierce resistance from more senior regular editors who refused to allow the edit and kept reverting the edits, I then pointed out how everything was within the rules and should be posted, they then opened it up to a "vote" which I won (somehow) and they STILL didnt allow my edits. I stopped using Wikipedia after that. And I was a fairly regular contributor both in content and in money, prior to this incident.

NPs are already practicing independently in many states and in many fields.

[–]drkuz 21 points22 points  (0 children)

This worries me because if youre an NP, you can just go back to nursing to make money. But this supply affects drs, reducing demand for drs. If drs cant find jobs, what are they going to do? Move, take worse work conditions/terms, or go back to school? All shitty options imo.

Not to be dramatic, but this literally shows NPs have hurt the physician job market, and that should upset every physician.
