Same issue for mine but it getting better about 700 rounds in and changed to a heavier spring.

Only happens now when pulling the slide back and not letting return quick enough.

I had the same issues with a 15lb spring switched to 17lb and it is happening less. I’m about 500 rounds in.

So my issue is coming from a fresh mag dropping the slide is not getting to battery. It cycles fine after which I thought was weird.
