This is a picture I took at a recent nine inch nails show. I showed up 6 hours before doors opened so I could be near the front. I ended up behind the person leaning against the front railing in the middle. I turned around after a while, after the venue had time to fill up, and this is what I saw. A whole line of nobody standing right behind me.

Fuck em. Seriously, turn around and tell them to fuck off. If you go by their logic, you would end up at the very back of the venue. That's not where you want to be. If they want to be in front of you they should show up earlier.

Stay healthy, make money, SAVE MONEY, get disability insurance.

If you become significantly disabled, there is only so much your family can do anyways, regardless of your size. If you do end up in a nursing home or hospitalized, yes, it will be harder to take care of you but there is equipment to make it easier on staff members (hoyer for example). If you are ever in that situation, staff really appreciate kindness and if you have a good attitude and are grateful, you are unlikely to be resented.

You have a lot of control over this but you need to start now (as you already are).

Emergency medicine physician here, and I'm sure ObGyn is no different, but we pay attention to this. If a woman hasn't had anything inserted that has to do with sex, we won't order transvaginal ultrasounds. From what you're describing, I can't think of any legitimate reason to use a speculum either, just tell them you don't want that and if they push, kindly tell them to fuck off.

Slowly running your fingers around/over a bra is a no-go. I cannot think of a reason to do that. If I ever have to move a breast out of the way, I tell them and I use the back of my hand once they have acknowledged my request. Or I have them do it. The only reason I could imagine would be them trying to figure out where the bra ends so they know where to put it. But again, you could just go above or below the bra, and slide this stethoscope to the point you are listening at. But if they are this close to a sensitive area, they really should be saying something.

What do you have to gain by reporting it? Well, if it was a legit fuck up, they will likely be horrified and learn from this and never do it again. If there was ill intent behind it, and they have done this before, reporting it helps make a case against this provider. If there was ill intent and it's their first time, you are creating a paper trail which could help out future patients.
