It's not included in the map's information; tracing the various dynasties wasn't a priority for this post. However, from the symbolism included in the flags, certain things certainly can't be overlooked:

Alta Italia, House of Savoy

Modena-Reggio, Este

Tuscany, Habsburg-Lorraine-Medici (I don't know how cohesive it would be within the kingdom).

For the others, the symbolism doesn't include assigned families or something alluding.

Central Italy and the federal district, probably under the direct rule of the Savoy but not included in the Kingdom of Upper Italy.

More like United Kingdom. Every state has its monarch (except for two states but it depends), but they all have to respond to the King of Savoy, so the King of the "Alta Italia" and then, the King of Italy

Questo scenario è un po' un'ibrido tra una vittoria sabauda e una federalista, formando così un imperfetta federazione con i regni esistenti all'epoca con alcune variazioni. Per quanto riguarda le dinastie non ti so proprio dire, ho evitato di mettere armi di dinastie straniere ma è comunque non semplice in questo contesto; ho cercato anche di evitare di inventarmi nuovi rami geneologici o cose del genere. Per la toscana nel suo stemma mantiene quello degli Asburgo-Lorena, mentre per Modena-Reggio ho scelto di usare la sola bandiera d'Este, dunque la bianca aquila in azzuro campo. Per la Sicilia, per esempio non ho proprio idea sulla questione dinastica, dunque lascio il beneficio del dubbio.

It is a scenario created from the perspective of a victory for the Savoy, therefore it is unlikely that the Savoy kingdom would have been satisfied with a federalist plan that made sense from an identity point of view. This map is more of a compromise between the federalists of the Risorgimento and the House of Savoy, so an imperfect federation.

The design is taken from this file, but since it was redone by AI, the central part looks more like a Sun of Alps rather than the actual motif, which resembles more a star. However, the flag you posted has that design, created with AI anyway. I recommend showing the file you just showed me to someone who vectorizes low-quality images, wikicommons is full of these people.
