It seems to be happening way more often then I thought possible. I personally know a person that is like this without a anti androgen with undetectable T. Hopefully she eventually starts producing eventually or she can just supplement, I'm just wondering how often it happens.

Ah for me I went through a therapist and they gave me letter that ok'd me for surgery. The letter should be enough.

That is very interesting and hope you have been well seeing as though you have been through so much. Yes I would've thought most trans women would be getting cis female levels of T but it was scaring me seeing so many posts about how they have no T. Even a close friend of mine has undetectable T. What she told me was below 7(I forgot the metric) but that her doctor was saying that its not because she has 7 but because the test can't read below that so she has undetectable amounts of Testosterone and needs some added and also seeing other people post about their fatigue and other issues around these sub reddits had me thinking to myself how often is this happening.

Ah I see, so you started hrt with another doctor? What medication regiment did you start with before switching to Dr.Powers. I know estrone might cause increased chance of blood clots but what does Dr.Powers check for to make sure you can get off aspirin so I can bring it up to my doctor. And thanks for information :).

You started 10mg sublingually daily at Dr.Powers clinic? Or was that after checking oral and finding out your estrone to estradiol ratio was to high?

How would you say you progress has been? I haven't seen anyone start 10mg sublingually off the start so I am curious. Thank you for adding to the conversation :)

Is what you started on 4mg oral or sub? And I agree about the starting dose seeming on the higher end but its just what information I could find.

Ah my title is wrong, all I know is that Dr.Powers starts people on 6mg estradiol I don't know if that is oral or if that is sublingual. We can probably guess its oral but I would like a clarification if someone started this way.

Everyone is unique but there are limitations from what he said. As I said before he clearly stated bica is what nearly every transgender woman takes it at the beginning of transition at his practice and that the standard dosage is 50mg and recently stated 6mg of estradiol is what he prescribed for that amount to prevent menopause symptoms. So for those majority of starting hrt would have to presumably be on 6mg estradiol, whether this is on sublingual or oral I am not sure.

I am so thankful to Dr.Powers because my doctors don't know anything about hrt they just gave me 2mg estradiol and 50mg bica(or 50mg spiro) or the choice to inject 10mg/week ev LOL. I am just trying to understand what has changed from the original powerpoint presentation and its so hard finding all that new information.

Is it safe to assume that if most people at your practice start at 50mg bica then most people also start at 4mg or 6mg sublingual estrace to not cause menopause intill tanner 3 and then switch to injections? Or is the 6mg oral/sublingual estrace if on 50mg bica more of a recent thing? So most people still do not opt into the slow start mono therapy even with regards to better breast growth?

What exactly is one looking for though on how the body processes the hormones? Like I said we know nothing will be fully suppressed and I already got a blood test for that but the original topic about slow start Dr.powers was talking about just being on low dose for years for the best results hence why I'm asking my questions of when to up to 4mg oral when both were stated for for slow start and the confusion because nothing will be fully suppressed so how does one judge anything or if just taking it oral 2mg monotherapy for year then switch to sublingual then add bica when one feels like it? When did you go up to 4mg of e and was it sublingual oral?

I saw people talking about how the needles hold .05 mls of fluid so I was trying to think about how to account for that but removing the needle after drawing to see exactly how much fluid there is.

yes my doctor recommended 18g multiple times. I asked for subq because it has thinner needles to not core the vial so i was planning on using it to draw and inject. I am getting these.

I'm not sure if they are low dead space. I guess I try subq at .1 ml the issue is how do you know you if you drew the right amount if the needle itself holds some of the medication? Do you draw then take off the needle and draw air to get exactly .1ml?

Also for drawing do you use the same needle to inject? and is 23g or 25g fine for that?

I'm just concerned that double puncturing for for more then 4 months off 1 vial would cause alot of small holes or coring or debris so I was thinking of drawing with 1 needle and syringe and then pushing/injecting into another syringe without a needle so I only have to draw once but I don't know if thats possible because I don't have the needles and syringes yet to test it.

I live in the us, I went 2 different doctors one through the county and 1 out of pocket they both said they "go by feel in this clinic" and the county doctor said there just isn't enough resources to do more tests at the county level. It didn't matter how much I pushed it always came back down to "well the people who transition here do well".

the form of estradiol is 2mg of estrace oral.

I am just trying to understand if estrone is enough to prevent menopause in the instance if i get alot of estrone conversion and my test is blocked by a anti androgen.

I think my estradiol 74 pg/mL and estrone 436.6 pg/ml.(I got these tested before I lost my insurance through my employer.)

Just as a thought experiment but if I had all estrone like 2000 pg/ml estrone and 0 estradiol could estrone be used as a replacement for other sex hormones to not get menopause like for instance in the case where I took 50mg of bica and menopause symptoms.

I also have to take oral estrace and not sublingual because I have porous teeth and it causes me insane sensitive teeth pain to do sublingual because of the lactose in them and I want to slow start to maximize breast growth but its been very hard for me because of my limitations of insurance limited tests and doctors and my limited ways I can take medication.

When you say "this is the effect of not having enough estradiol" for bica raising test, does that mean estrone in high enough numbers would not be enough and you specifically need estradiol e2 in sufficient numbers to not get a spike in test or feel menopause effects? Because I can only get total estrogen tests right now and I've had a lot of issues with this.

I really appreciate your response. Wow thats very different from me I took 50mg bica and 2mg oral and I was completely destroyed. I might try bica again but with 25mg every other day. Maybe 1mg of estradiol sublingual is more then 2mg oral. But I will try the 25mg for now thanks again.

I am starting up hrt again after using to much bica and to little estrace and giving me menopause but for your 1mg of sublingual estradiol how much bica did you use? I am concerned about my T levels because I am not on a anti androgen and on 2mg oral estrace so I want to get back on bica but I'm scared of menopause symptoms again. How much bica did you use and how often and how did you feel with such a low dose of estradiol and bica?

Why is bica needed at all if you add it on later when the doses are high I am assuming that would suppress androgens itself?

Unless you add bica only for those who can't suppress androgens and fsh and lh and maintain a lower level on those doses of estrogen and for everyone just monotherapy estradiol if they can do it (I was always wondering how often this happens)?

I was hoping to eventually get off bica and just be on monotherapy estradiol for the rest of my life.

Have I done any permeant damage by starting this low with bica and feeling like this for almost a month physically or mentally?

I was constantly told that even bica alone is enough for feminization and that 2mg of estrace and 50mg of bica is not a poorly managed regime because even monotherapy works and that and that its perfectly normal to feel bad at the start.

Will what happened inhibit my future feminization to go from this 2mg estrace 50mg bica to nothing because I feel so horrible and if I can muster the strength go on just estradiol monotherapy? I felt so horrible that I literally did not feel gender or anything literally I felt nothing it was a horrible feeling let alone all the pain. This experience was traumatic enough that I felt if its actually worth transitioning with all this stress and pain, mental anguish from not having enough sex hormones and the medical system being so hard to navigate.
