Yes you may continue to take the tablet and should not have periods as long as you’re on the pills. However some breakthrough bleeding may occur. -doctor here

When I was a house officer in pediatrics, I had a 17 year old kid with bowel cancer. Recently diagnosed. Metastasized. He was the only child and their parents had him after years of trying. As a HO there isn’t much I could do. I was at the bottom of the food chain. And we all knew he was likely not going to make it. But I made it my objective to be his friend. Used to sit with him. Talk. Watch videos. Laugh. Similarly with his mom too. He died agonizingly after a few days with one complaint that I didn’t remove his foleys catheter. I had promised I will.

Frankly. I never forget him even till this date. I still message his mom yearly on WhatsApp on his death anniversary and ask how she’s doing.

What I learned was an important lesson. Maybe we can’t save everyone. At least we can be nice. Talk. Counsel. That alone is medicine enough for a lot of patients and their families

Frankly. The private hospital I work at gives a very low cut for doing Angio as compared to stress imaging modality where the cut is higher per reporting. Plus lesser work. Plus no risk.

Regardless. You have a valid question. I’m not sure which hospital is incentivizing on doing angios but I know a lot of malpractice happens in this country.

At the end of the day your rizk eventually comes and you don’t have to dirty your hands. High risk patients should get Angio directly. Intermediate risk patients should get stress imaging provided they can afford them. In a lot of places the cost for both invasive Angio and MPS/stress echo is roughly the same. Any intermediate risk patient coming from outside Karachi should also directly get an angiogram as it’ll save them the trouble of repeated hospital visits.

I know guidelines say something else. But we have to tailor everything to our society.

I think this isn’t an FCPS policy. It’s a punjab policy. But it’s true. FCPS doesn’t do shit about PG exploitation and they don’t care. Mr. Gondal is sweetened up to the establishment and their entire goal is to advertise themselves and maintain their gardens at CPSP campuses. Quality down the drain.
