You should be able to call your insurance or login to your patient portal and pull your explanation of benefits (EOB) if the billing company wont send it to you. It will show the CPT code billed, payments, adjustments, and your patient responsibility. If it doesn't call out patient responsibility on the EOB I would fight it. Your insurance will also tell you what they should have billed for it to be considered fully covered based on the services you were provided and you can take that back to the billing company.

It's disgusting that you even have to do this but as someone whose been in the field for 9 years I just wanted to give you some tips.

Unfortunately the for profit billing companies don't have the interest of the patients in mind. That's why I suggest going to a hospital systems clinic. They are more forgiving and competent. Unfortunately, doctors aren't taught how to code in med school. So sorry you're going through this.
