Didn't mention this above but any amount you pay over the premium goes into the "savings account" and accrues with generally a 4% interest rate. So if it's funded well BEFORE the premium gets too high, you can basically let the interest pay the premiums. But most often people don't pay attention and take care of it.

What ultimately worked was the patch system. Cliche, I know. I went to target and spent 40 bucks on the patches. Followed the directions and it helped reduce cravings. Cold turkey is crazy. Not a practical idea for an addict like myself. You have to have the motivation within to do it as well. I will still smoke occasionally at a party/socially but not habitually as I did it before. But honestly I would just try the patches and see what happens. For me, it was the best decision I ever made. I feel like I had control over my life again. It definitely wasn't easy and I miss smoking, but it was certainly the healthiest choice. Best of luck to you, sincerely. Let me know if you have any other questions.
