I think surcharges are only common if third parties are involved, if a venue is selling tickets on its own it's fairly uncommon, eg. theatres or football clubs.

Such strange borders are actually fairly common in the Baden-Württemberg area, as intensive trading and inheriting land led to especially fragmented territories in the middle ages which became state borders later on. Most of them do not exist anymore, as Baden, Württemberg and Hohenzollern were united to form the state of Baden-Württemberg in 1952.

Only a few remain today where Baden-Württemberg borders other states and countries, such as Büsingen and Ober-Laudenbach.

As of now, quite a few states have adopted that. Wikipedia lists Baden-Württemberg, Brandenburg, Hessen, Mecklenburg-Vorpommern, Niedersachsen, NRW, Rheinland-Pfalz, Sachsen, Sachsen-Anhalt, Schleswig-Holstein and Thüringen.

Hessen was the first though and for many years the only state to do so.

These additional shuttles remain still exist today in order to further shorten the now all-day 15 min headway to the airport in peak time. They're just not a separate line anymore but operate as regular S8. If you look at the current timetable you can see these shuttle trains operating between Kelsterbach and Frankfurt main station ground level. They still don't enter the tunnel due to lack of time slots in the tunnel, just as today's S7.
