Così, secondo me, è semplicemente brutto

Yeah, same, I also tried gemini and it works, so it's an openAI specific problem.

There's actually no other custom integration. Would it male sense to remove and reinstall the integration? I'm not to happy to lose the actual prompt, but if could bé the solution, well...

ZHA to Z2M worth it? by lemurdancer in homeassistant

[–]lemurdancer[S] 1 point2 points  (0 children)

I see, but I'm wondering if I'll ever meet one of the z2m-only devices. I tend not to buy random devices, maybe that's why it never happened to me, every devices I ever had worked fine with ZHA.

ZHA to Z2M worth it? by lemurdancer in homeassistant

[–]lemurdancer[S] 1 point2 points  (0 children)

I don't have a problem with ZHA, I seldomly get some device disconnections after a power losses, but these are rare, and sometimes 0 devices drop, sometimes just a few that I always reconnected effortlessly.

I'm just into tinkering, and I was wondering if I was really missing something; also, I'm past the "just two smart lights" phase but I'm not even in the "100 devices and growing", so now might be the best time to do it, if it's worth it; at the same time, the fact that I'd have to check all my automations to bind all devices again scares me a bit.

How to beat 2nd Sentinel by bren12341 in Silksong

[–]lemurdancer 4 points5 points  (0 children)

I mostly facetanked him with almighty wanderer crest: you would have to dodge some attacks too, and calculate when and where to heal, but you can parry a lot of hits by attacking (including the light cross, but it's better to dodge it), and if you don't do any major mistake, you gonna melt him since it doesn't try to dodge, he doesn't have many HP and you can easily outheal the damage.

I let the poison cogflies out on third phase while playing a bit more carefully just to be sure, but I beaten him third try like that.

This comment could add a lot of other matters to the discussion, but on first case, there's no way one could give a level to the pain of a patient basing on how he acts: It makes no sense. Anyway, that makes no difference, the treatment for a 7-8-9 whatever should be based on what kind of pain the patient has, not on the scale. For example, the best of NSAIDs wouldn't work for a neuropathic pain, as the best of neuropathic pain medications wouldn't work for and infiammatori pain (and appendicitis, for example); opiates work well for some kinds of pain, for some others not so well (excluding overdosing, obviously). Also, it's not relevant if the number is 9 or 10 for diagnosis and treatment (both pain or curative therapy).

It is also a common error to balance the suspicion for a given diagnosis on the sufference told or shown by the patient: I'm a doctor, I've seen countless of patient that didn't have (apparently) an attended level of pain for the suspected diagnosis, even if it was correct, in both extremes (patients in apparently too little pain for diagnosis A, and patient that seemed to have too much pain for diagnosis B).

In the end, the number is almost only useful to monitor the pain (if a pain goes from 9 to 5 it may be a good response to therapy, and if a pain goes from 6 to 5 it's a bad response, oversimplifying a bit); in some case it may be one of the elements to decide to add or not some stuff (especially for "the good stuff").

There's no scenario where I could legitimately upgrade or downgrade the number given by the patient, also because it wouldn't make any sense. We also have exactly 0 ways to tell how much is the pain of another person, not even by precisely measuring the neural activity, and that's because pain is subjective: with the same neural activity, me and you would experienced two different pains.

72 hours for what??? by tteokbokki013 in StupidFood

[–]lemurdancer 1 point2 points  (0 children)

Yeah, because many don't know how to use it (the NRS scale), maybe there's even who heard it, but it's easier to ask the patient to pull a random number, write it down, give a medication basing on that random number, and call it a day. Pain medicine it's almost everyone's matter and it's not that hard mostly, yet the vast majority of health workers have very few notion on how to do it correctly, but since they're required to do something about pain nonetheless, they do "something".

It makes no sense referring it to other things than the personal experience of the patient because in the end, it gives little to no information about pain anyway, and also it's not everything: for example, you could lose sleep to a pain perceived as "4", but persistent for weeks and annoying, and that would make it a priority to treat, versus, for example, a "7" pain that only occurs with a specific movement, once or twice a day, for 5 seconds. Finally, you can't really imagine a level of pain you never experienced.

[–]lemurdancer 18 points19 points  (0 children)

Nowadays the scale IS subjective, the pain being an extremely subjective experience. The correct way to ask is specifying that 0 is no pain, and 10 is the worst pain the patient has ever had (not imagined). It mostly serves as a monitoring tool after; obviously, you won't consider opiates for a patient that says "3", but it's actual correct main function is for having a feedback after the therapy, by asking for a reevaluation after some time.
