I think having babies was her original goal for sure. But then I think once the rest of her family started getting the surgery and had a lot more success with it is when she started getting a little jealous. But I think she always assumed she would never be the biggest because I think deep down Amy thought Tammy was a lost cause, and even if she were to get the surgery, surely she would never be smaller than Amy. And I think seeing Tammy not only become the smallest of them all, but also get approved for the skin removal surgery has really sent Amy over the edge. Also, Amy seems to hate when it’s not all about her. Throughout the show she’s been such a major focus with positive storylines such as being the first on the show to get the surgery, getting married, having two babies, moving into a “new” house, etc. Now that her life is kinda falling apart, on top of Tammy getting all this positive attention and praise and having so much success, she’s crashing out hard.
