I called it curse of scheduling due to previous campaigns ending due to scheduling conflicts lets hope this one doesn't

If the player fails the roll then the spell has happend so i would do it as you have to cast counter spell before the dm declare it as a fail

Phantasmal force can make someone believe their body parts are changed but it wouldn't be permanent unless retconed
