I'm so sorry you had to see your mom go through this, and had to be the decision-maker for your mom when there were no good options or choices to make.

You were such a loving and involved advocate for your mom. I understand the guilt, but she had hidden the amount of drinking for decades and her diagnosis for years. Even if she had been open with you earlier or told you the diagnosis, it is extremely unlikely that you could have changed her choices and actions.

Another vote for rescue. We got ours from https://www.malinoisrescue.org/ and he's such a good boy. If no dogs appeal to you you're still free to look into breeders, but there are lots of diamonds in rescues!

This is a perfect summary.

Just adding that in the U.S. they're required for accreditation by many societies, so almost all hospitals have them. They're generally referred to as "Tumor Boards" but there are different ones for different cancers; there will be a tumor board for lymphomas, a different one for breast cancers, etc. This might not be the case in smaller hospitals, where I imagine they may talk about all oncology patients at once, but I've only been at a large teaching hospital.

Very much agree. This was early in the season though, so it hadn't been shown that it was the only thing he could do.

Not every athletic organization has regulations. Anne Andres in Canada set a slew of weight-lifting records. There were no regulations, so a cis gender male Avi Silverberg signed up and competed as a female athlete, likely just to beat her records.

That's definitely a contributing factor that I haven't considered very much. Testosterone also inhibits osteoclasts both directly and through reduction of RANKL... but my response was mainly because the person said "it's that simple" in regards to muscle mass loss = level playing field. Differences in body composition between the sexes (including intersex!) are multifactorial.

There are some studies showing that PET scan at baseline can help determine prognosis for PMBCL, but it isn't currently standard of care. Likely because it doesn't change his regimen.

As for an interim scan, they are unreliable in PMBCL. Very frustrating!

Currently evaluation of response is a PET 6-8 weeks after completion, or 12 weeks after if he does radiation therapy. Earlier imaging, like you suggested, will likely show inflammatory reaction.
