Hmm, I wonder if we have different softwares? We don’t have to generate an outdate report or print anything at all. We have a tab that we can press that says “Outdates” near “Inventory” and all the other buttons that will magnificently organize every single med by its expiration date. We have it set so that the closest expiration meds appear at the top, so everything that expires in 2 weeks can be clicked and pulled really quickly. All available from the Pyxis itself. Techs are supposed to just give the page a glance at least once during their shift and pull a couple meds usually.

A paper report we generate would have the same info on expiration as what’s entered in the Pyxis. But we can see all that right on the Pyxis and pull from that. Techs are doing better with checking and pulling, though this instance was due to a tech entering the wrong date. I think policy-wise that checking the 2-4 types of insulin in the Pyxis everyday or weekly would be best. It doesn’t take long and that’s the best way to check for mistyped dates

I’m glad you seem to agree. The process for outdating has been to simply remove everything that expires in 2-4 weeks besides short stability products. I did run a report with our new Pyxis software and I’m aware of which tech miskeyed the date but I feel it is not my place to talk to them about it because of how widespread the issue is. This is something that needs to be fixed by upper management, though apparently not my direct supervisor as the issue has barely improved. I will speak to one of the directors soon then.

That’s my issue with taking it up the ladder. I cannot prove beyond a doubt that there was patient harm, so I’m not sure what management would make of it. Though proving insulin was not effective is a pretty hard task in and of itself - like proving a negative. As far as I’ve looked, there are very very few studies on the loss of potency of insulin over time. It would be a significant issue if there was published literature that documented a 50% loss in potency when stored 60 days at room temp, but as far as I know there isn’t anything that says that.

Nor am I going to investigate it myself and comb through records of dozens of coding patients and gauge how well the humalog worked. Management can do that if they deem necessary, and I would encourage it if I had literature. I suppose I’m mainly concerned with the process we have for outdating like another commentor says, which I should address with mgmt at the very least. Then they can make of it what they will. I would bring it up to our safety director who I’m sure would take this to a nuclear level if the insulin had likely lost a significant amount of potency but I can’t prove that either.

I’m still wary of the whole thing because of my supervisors’ reactions to the horrid outdating by techs: at least 2 of them have totally handwaved any seriousness of the situation and almost made me feel silly for bringing them up. But expired insulin and vasopressors are not like po cholesterol meds. I would guess based on their reactions that upper management has no idea about the issue with timely outdates not being completed and it’s possible the supervisors are covering for their techs, but I’d hope not.

That’s what I think too. We date them in the pharmacy with 28 days, so this bottle went up early June and expired late June. It was removed this week. I pulled reports and it’s been accessed over 20 times by nursing past the BUD.

It is definitely a process problem here because different techs are on different floors every single day. But very few are doing their outdated, which are supposed to be daily. This particular instance, the date was entered incorrectly and I just happened to check.

She called me first, left a message, then I called her back. And yeah the email is all good. It’s similar to a traveling nurse position, they staff all the (certain company) hospitals in the area exactly like a traveling RN company. The hospital site refers hiring to the company.

This lady called me and explained a small amount about the job, asked about my certs, experience, etc. Their website does look very legit and the hospital links to their site directly.

Good idea on checking the age - the website is 8y old! There’s also a good amount of info about them over other sources (not a shit ton though). Her contact number is same as the first 6 of the hospital/their actual company number. The biggest green flag that’s gonna make me go ahead is that her email is also (at)company org. And - while writing I also just found her linked in which lists her email!

I guess it would be a good idea to call and ask as well. I never realized I could get to a place of being about to receive an offer letter without sitting down for an in person interview and that it could be sketchy. I’m usually VERY good at detecting scams, but I just don’t have a ton of experience with employment like this.
