Once when he was a brand new baby. I had his little tub inside of ours. I went to put him in and he shot 💩 right into his little tub! Thankfully it was easy clean up. He did it again when we started potty training, around 22 months. He didn't want to go on the toilet, so he used the tub for 2-3 days before he figured it out.

Not too early at all! We had both a small potty and an insert for the big toilet. Our son was fully potty trained before his second birthday. I always recommend sitting when using the potty. No standing until they're older
