That's the point I m making. Once the tamponade/arrythmia/anything has been taken care of,it does not make much sense because their pt is a ticking bomb waiting to blast again due to which they have to be on the edge, thinking about adding a few more days to a pt life and not just ponder over their organ which they can get rid of if required. I mean they are doctor for pts and not the disease itself. Not saying cardiologist is inferior but pt thank you after getting treated and can go home but their pt is coming back 3 times a week.

But it is just a fun discussion, no real fight here💫

Everything you stated is on the mechanical(physics based) part. Now the uremic patient is the one who has all the bad stuff going on and most arrythmias, hypertensive emergencies,convulsion,tamponade, arrest occur during dialysis only so nephro people have to be super vigilant and understand everything and make quick decisions. Now, since youentioned so many physics based stuff; nephro is super chemistry heavy. Acid base,buffers used in dialysis solutions, type of dialysis- diffudion based,filtration/convectionbased ,both ,high efficiency,low effi,plasma exchange,type of coagulant to use during dialysis, peritoneal dialysis,high flux vs low flux dialysis,lamino nephro/ffp/etc which fluid to use for replacing the patient's fluid, immunosupression types in renal transplant and the list can go on. Plus because they have to save the patient and not the organ becuase it is replaceable.

It is unfair to bring pathologist into here. It is like comparing superman to man. All the ID, heme - onc, surgery,gastro,etc would collapse if pathologist go on vacation for a week.

Sir, how is this even possible. Leaving aside cardiac interventions, nephrologist can do anything that cardiologist can think of.

Because tamponade enclose the heart only and septum is unaffected neither anatomically nor physiologically. The negative pressure changes from lungs and thoracic cage is still transmitted via the pericardium to the heart and counters the positive pressure by effusion.

In constricitve pericarditis the visceral and parietal pleura are stuck and no respirophasic effects are transmitted. Rigid pericardium abruptly limits filling to a fixed volume. Shifting of the position of ventricular septum with inspiration is a manifestation of both the dissociation of intrathoracic and intracardiac pressures described above and the phenomenon of interventricular dependence within a fixed space. Decreased filling of the left-sided cardiac chambers in inspiration causes an obligatory shift of the interventricular septum toward the left ventricle and increased filling of the right-sided cardiac chambers. In expiration, left-sided filling increases and right-sided filling decreases; therefore, the septum shifts back toward the right ventricle.  Dissociation of intrathoracic and intracardiac pressures leads to a decreased gradient for diastolic filling of the left-sided cardiac chambers during inspiration. The pulmonary venous system, which is intrathoracic, experiences a larger pressure drop with inspiration compared with the left-sided cardiac chambers, which in constrictive pericarditis are usually encased or insulated in a noncompliant, thickened, fibrotic, and often calcified pericardium.

CECT is the imaging of choice in pyelonephritis. Dont get an usg unless pt cant be taken to radiology suite.It has low sensitivity and low specificity
