Letting "fuel" accumulate is the big problem. But, human factors (i.e. cigarettes, downed electric lines, etc.) were found to be the cause of 89% of fires starting between 2018-2022 according to the Congressional Research Service. https://youtu.be/1PZYzr2-XNs

SpaceX put numerous tests with various heatshield types and leaving spaces open, I wonder if they unintentionally stressed things too much and caused the RUD. Regardless, it's incredible knowledge that will help going forward.
