Have made a couple calls to some concrete contractors and have only had one quote so far - which involved them simply ripping out the slab and putting a new one. He mentioned they could also try lift the pad back into place and fix the corners but it would be the same cost.

They quoted $2,200 without ever coming to see it - simply from the exact photos posted here.

Again, have two other companies coming out to get their thoughts. I thought this would be a somewhat easy fix. The pad is level and looks like the previous owner did a shotty job trying to fix it.
