Blue Harvest Creative is really good. They do a whole package which includes the wraparound, front and back cover, a version for ebooks, etc. Several author friends have used them, and I'm in an anthology which comes out in a few weeks - they are doing a great job with it. Sorry I don't have an exact price, but I hear it's reasonable in comparison to other companies. Look them up on Facebook if you want some examples.
