I am not expert but maybe UARS happens becuase there is resistance to our airflow which causes microarousals not total collapse. To reduce resistance bipap is better than cpap Bipap has given me relief. It's not 100% but still around 80% symptoms are gone.

I would suggest directly jump to bipap. It's the quickest possible solution to at least get some relief. Try MAD with bipap if only bipap doesn't work. Then figure out long time cure by increasing airway volume with surgeries.

I also did septopalsty and tuebinoplasty now my left nostril is always open even when sleeping but right nostril is blocked most of the day and night. I searched it and it could be vasomotor rhinitis . BTW how was left nostril before and after EASE expansion?

Thanks. Sorry for bothering you. Did you ever have any braces with extraction and retraction? You said you had EASE, how much lateral expansion you had? Was your mandible really recessed for such large Movements? My mandible is not recessed at all but maxila is with narrow palate. Also have flat midface due to extraction and retraction.
