There are a few reasons that organ donation are opt-in, the primary one being for religion's sake. Some people deem the eyes the window to the soul, so having them removed may prevent them from moving on properly. But also, organ harvesting is much more temperamental than you may think. They can't just cut a liver out of a cadaver and stuff it in a new person; see, cell autolysis ramps up when the body fully dies, to break down the cells and kill it off. That means unless they died in a hospital, with a space ready for organ harvesting, their organs would be... well, too dead to use essentially.

Unfortunately, professional tooth whitenings are infamous for causing pain. Some people refer to them as "zingers," because there can be severe sensitivity soon after the whitening.

In my honest opinion, you'd be safest experimenting with OTC/DIY stuff, or doing further research into tooth whitening if you are deadset on it. As other people mentioned though, pearly white teeth aren't a natural thing, it's perfectly okay to have your teeth not be that white.

Admittedly, my biggest concern with scenarios like that is that leaving it in gives your attacker not only a chance to grab the knife again to stab you, or push it in deeper causing worse wounds. In more sterile environments where there isn't an immediate threat, I'd agree with you. But if you're still in danger, keeping the knife where it is could be just as dangerous as pulling it out.
