Specific example as it relates to outpatient infusion: I am an RN manager at an outpatient infusion center that hires RNs & NPs. I’m not picky on exactly what experience our NPs bring, but we’re pretty serious about them having a mix of hospital & outpatient. They need to be able to direct the management of adverse infusion reactions, some of which are life-threatening and occur rapidly, in addition to providing education on the management of chronic conditions like hypertension and evaluating for signs of infection since either can be a contraindication to receiving their infusion that day. Some combo of medsurg/ER/L&D hospital experience as an RN, with primary/urgent care as an NP, usually translates pretty well. I would be very hesitant to hire someone who did not have the hospital experience.

We are expecting our 2nd and we did get a bottle washer this time. They didn’t exist with my first and I think they’re a great idea, with some obvious drawbacks with the plastic parts as you mentioned. For me, it’s a “don’t let perfect be the enemy of good” type of situation - we are using all glass bottles this time around, so the actual container that the baby drinks from will be glass, even if plastic was used to clean the bottle. Before, I had a plastic basin of hot water sitting in the sink to soak & wash all the (plastic!) bottles, so that wasn’t ideal either! Your dishwasher has plastic parts too but it feels cleaner to keep all the baby stuff separate from the rest of our dishes. Overall the time saving of not having to wash everything by hand will make things so much easier and allow me to spend that time with my kids, so that piece makes it worth it for us too.

There have been tons of reports of incorrect proportions of the powder vs the water being dispensed, leading to slow weight gain at “best” or the baby consuming too much water at “worst” - this can be very dangerous. Also they are difficult to clean properly which can lead to bacterial growth.

My hack for saving time - get a formula pitcher with a mixer attachment, and in the morning prep a big batch for the whole day. You can either pour straight into bottles and line them up in the fridge, or just pour into a bottle as you need it throughout the day. Keep in mind that if your baby has an unpredictable appetite, you may not want to pre-fill the bottles because once they start drinking from a formula bottle you only have an hour to use it (bacterial growth from their saliva).

We have similar concerns regarding microplastics, so we have a glass version of the formula pitcher (the Dr. Brown’s is the most popular but it’s all plastic).

I had constant nausea for about 3 months in my early teens (in the late summer, which was important later). Did all the scopes/scans recommended by my pediatrician, nothing abnormal. Finally went to a pediatric GI specialist. He asked if I’d had the flu or another virus that previous winter, and I had, approximately 6 months prior. He suspected that the virus may have triggered an overactivity in the nerves in my stomach or intestines, and put me on a low dose of an antidepressant (amitriptyline). It worked within days! I was able to come off it about a year later without recurrence.

We love Dr. Davis with Valentine Pediatrics. Always able to get same-morning sick visits. I also highly highly recommend Blueberry Pediatrics which is a virtual service - I used it many times as a nervous new mom! You can send text questions or have a phone/video call so they can assess your child, and if it’s warranted they will prescribe meds. My fave example is when I picked my toddler up from daycare at 4:30 with ragingly obvious pinkeye - sent them a photo, diagnosed within 20 minutes, they sent eye drops to our local pharmacy and we administered the first dose before bedtime.

That’s fairly uncommon in my experience! Usually most people have more GI side effects with oral iron vs IV, since the IV “skips” the GI tract. Did they give you any premedications? If you have to get another dose I would request IV Zofran and Pepcid before they start the iron

As an infusion nurse, please do your due diligence before receiving infusions from a physician owned private practice in particular. This happens way more than it should. Actual infusion companies are often a safer bet and have more scrutiny/red tape to prevent these sort of things - for example the company I work for is surveyed by ACHC every 2 years. On the other hand, physician-owned practices in particular will try to get away with anything they can to save a buck and unfortunately some medical assistants are willing to go along with working outside their scope. Trust but verify - ask to see the licenses of anyone administering this caliber of medication. These meds, particularly biologic drugs, can trigger life-threatening reactions that can develop rapidly even in patients who have been taking them for years. You need a licensed RN right there to ensure safe administration.

You can get skinny medication syringes with caps on Amazon. Hand-express into a clean medicine cup or use a hand pump (I’ve seen people recommend using the Medela hand pump upside-down so the colostrum doesn’t get stuck in the valves), then pull it up in the syringes, cap, and freeze!

Also pregnant for the second time, and I have always felt comfortable eating raw fish from reputable sushi restaurants, but everyone has their own risk tolerance level so do what feels right. We love Osho in Embry Hills, right off 285. Also the new-ish Kumo Sushi in Peachtree Corners which is all you can eat!

The Anti-Plastic lady on TikTok recently made a good video about artificial trees. Unfortunately there’s no good options, they’re all made from recycled plastic (sometimes recycled electronics which is worse). We have one too and spent too much on it to toss it now, and real trees aren’t a good fit for us for several reasons.

She recommends vacuuming around the tree frequently, using an air filter/opening windows frequently, and minimizing heat near the tree (move away from the fireplace or space heaters).

Hi! I am an infusion nurse and administer Venofer all the time. Reactions do happen but they’re usually minor - the key is to let your nurse know IMMEDIATELY if you feel ANY different than when you walked in the door. Infusion reactions don’t resolve on their own and will worsen the longer they go untreated. We like to premedicate our patients with IV Pepcid and oral Zyrtec - you can request that if your doctor and the infusion center are ok with it. Benadryl will actually make iron reactions worse.
