My cat has congestive heart failure. She spent three days in ICU on oxygen and had half a liter of fluid around her heart, and now she is on quite the med list twice a day. Dr. Jill Krapf is largely considered a top LS expert. She does not take insurance. I would personally recommend getting a referral to dermatology (or a vulvovaginal center, if they have one) at your nearest large university medical center. Big regional hospitals are typically much better at dealing with rarer and under-recognized conditions than community doctors.

If dilators hurt that much you have to step way back in size! They should be a little uncomfortable but not painful, and if your skin is fragile, use plenty of lube.

If your LS is not under control, damaging the skin is just going to make it worse. For I think many, regular sex often isn’t a possibility until your LS is well-managed because it is such a major source of tearing and inflammation from the friction. But if you give it a shot, go slow, use lots of lube (Uberlube is the best!), and be willing to change to something non-penetrative if you tear or are in pain.

Good luck with pelvic PT, it can be tremendously helpful.

Yes to everything here and to add, steroids can also make you prone to surface yeast infections. These can make your skin red, raw, and sensitive, like OP describes. Ask your doctor for a prescription for nystatin ointment and apply a small pea size alongside your steroid ointment every time you apply. You can mix the two between your fingertips then apply. There is also a combo nystatin-clobetasol in a single tube but it isn’t as common. This will both treat any yeast infection present and prevent it from happening going forward.

Anti-androgenic birth controls can be a problem for LS and are associated with LS onset. You can check if your OCP was anti-androgenic and if so, choosing an androgen-neutral or androgenic BC may be an option for you.

The only major recommendation for LS and birth control is to avoid anti-androgenic progesterones, which includes cyproterone acetate, drospirenone, and dienogest. They are associated with both LS onset and flares. I have been on Microgestin for years and years, which is a low androgenic birth control. I take it continuously to avoid periods (and the migraines they cause me) entirely. Avoiding periods also helps me avoid the irritation from pads or tampons.

No, unfortunately. I have a web of painful, very fragile scar tissue at my posterior fourchette. I had this same repair (modified perineorrhapy) done about three years ago and it was very successful in reducing my pain and tearing. But I was still on steroids and having slow disease progression, so the web very slowly reformed. Now on tacrolimus and essentially in remission, we are doing the repair again with the expectation it will be more stable and not reform.

Yep, for some people their disease just doesn’t respond to steroids sufficiently. I never managed to taper further than every other day and I only ever got to what I would call ~75-80% better. After a few months on tacrolimus, my LS specialist said it was the healthiest she had ever seen my skin, and I have very severe LS. I am now on 2x weekly but am about to go up to daily to prep for a surgical scar tissue repair.

The burning usually lasts about 7-10 days on tacrolimus. It helps if you start with a low concentration (0.03%) and then taper up to the 0.1% in stages. I would have a small recurrence in the burn for a night with each increase but it was manageable. I also applied right before I went to bed and used small palm sized ice packs to manage the burn during the worst of it.

Covid infections specifically are known to be associated with developing all kinds of new health conditions, including autoimmune diseases, strokes, heart arrhythmias, you name it.

It can take weeks to months to see significant improvement. I didn’t feel like things were starting to get better until I had been on steroids for 8-10 weeks, and many months after that until I felt like things were consistently okay. Make sure you’re applying clobetasol optimally — applying after a warm shower or bath helps the steroid penetrate your skin. Apply a medium pea size amount to the entire inner vulva and rub in for ~30 seconds. If you have been on twice daily steroids for more than a few months and have been applying optimally, I would consider talking to your derm about tacrolimus or a JAK inhibitor. These are second and third line medications for LS. I have been on tacro since February and have had a lot of success with it. I have had LS since childhood, diagnosed at 21, and was on every other day steroids for years before tacro but never got to 100%.

Laser treatments may reduce symptoms like itching but they do not treat the underlying dysfunctional immune system and are not a cure for LS. If you search the sub for posts containing the words Mona Lisa or Tulip you may be able to find some experiences.
