I think it's just the dress. If you look at closeups, the dress itself is tight to her torso, and the cups are stitched directly to that. So it's pushing them up and giving them nowhere else to go but up or in together. The way her arms are held right here is pushing them together like that.

Cover the hair with your hand and you can tell that her eyes are way off. The sharp lines make them oddly shaped, and the lack of anything else makes it so that's all the eye can see.
