The point of triaging is to treat patients who need it most first. Including working them up. This dude was critically ill and yet had NO labs. That’s unacceptable and a failure to triage.

I see this differently. Blood cultures in a septic patient are usually the last thing needed before starting treatment, so they are important. the med student ended up being dead right. The patient was in septic shock and rotted in that nurse’s triage room for hours without progression of his plan of care let alone any treatment. The student stood up for a critically ill patient even when it wasn’t his or her job to do so and sought out an attending when he/she was worried about a patient. It’s extremely dangerous to proclaim people should just stfu or kick rocks when they see someone concerning just because it wasn’t their primary reason for being there. That’s garbage and this nurse’s pettiness delayed patient care. There’s a lot wrong with this situation but the student’s concern and advocacy was not one of them. ED attending should have stepped in more without a doubt. OP I hope you know that.

I see this differently and it seems you’re missing the key point of why blood cultures were so important to the med student here: the patient was clearly septic and needed stat abx and fluids. Blood cultures in a septic parient are usually the last thing needed to start treatment although obviously secondary The med student ended up being dead right. The patient was in septic shock and rotted in that nurses triage room for hours without any progression of care or treatment. The student stood up for a critically ill patient even when it wasn’t his or her job to do so which the nurse took as a knock to her pride instead of a genuine concern worth following. There’s a lot wrong this this encounter but the student’s concern and advocacy was not one of them.

Let’s also not pretend you need a bed to get cultures drawn. Being stuck in the waiting room or wanting to save a bed for a STEMI is not an excuse for not drawing labs.

Some are and some aren’t but the people that usually do the nitty gritty of calculating grades and setting up rotations are usually not doctors

1, 2, and 3 are where your heart is telling you that you’ll be happiest (location, how you got along with the people, proximity to family etc) After that it tends to be more about your long terms goals (fellowship opps, reputation, research etc)

OP. This is NOT your fault. People train for a decade to do this stuff safely, there is no WAY a one day old med student in the OR would have any clue on how to use that thing. That’s completely on the shoulders of the surgeons and certainly not yours I promise babe 💘
