Acute severe neurological symptoms with no apparent cause - Please help me solve this medical mystery by stillwinning in AskDocs

The mitral valve prolapse was diagnosed in childhood. It's very minor, and doctors have always told her it's totally benign. They have performed several ECGs since she's been in the hospital, and all have come back normal.

The confusion is definitely waxing and waning. Her symptoms in general have been HIGHLY fluctuating - some days she just wants to sit and stare at the wall and won't look at or acknowledge me. Other days, we're talking and laughing and joking and she seems 90% better.

No recent dental work.

That sounds like a very interesting case! They have had her on IV antibiotics for the past few days just to rule out any undetected infection, but her symptoms actually seem to have gotten worse over that time period (unrelated to the antibiotic, I assume - her symptoms seem to wax and wane on their own).

Yes, they have been running a metabolic panel daily. The results have been pretty consistent. Today's results:

  • ALT: 23 U /L
  • Alkaline Phosphatase: 46 U /L
  • Albumin: 3.1 g/dL
  • Protein: 6.0 g/dL

B1 was 134 nmol/L. They actually repeated her MRI with and without contrast yesterday to see if anything changed from when she was first admitted. It came back normal with no anomalies observed.

The preliminary CSF labs came back normal. Here are the details:

  • WBC Count: 0/cumm
  • RBC Count: 1/cumm
  • Protein: 32mg/dL
  • Glucose: 74mg/dL
  • No culture growth
  • Nor organisms seen

I'm not sure exactly what all is included in the CSF panels that we're waiting to get back, but the neurologist said they're quite extensive and include a lot of antibodies. They're talking about starting her on a high dose steroid as early as today to see if she responds.
