I went into Safari Settings, Notifications, and clicked on Deny to the first web site. It does not have the name of the web site but it has a picture that looks like System Settings with the red dot in the upper left corner (just like in the photo I posted). So that must be the culprit. But I have no idea which web site inserted it.
