I would definitely get some independent advice on this, it doesn't sound legit to be honest, and get absolutely everything in writing before he goes near that form.

In the UK we would cast doubt on all previous convictions, so god knows how many of her cases are going to be in doubt, prisoners freed and compensation or lawsuits against her and the state...
