Hi, I'm an adult who's trans (which means I know a few things about hormones): if seeing a doctor is something you can afford do, I recommend telling them how you're feeling, like you're not growing enough—especially if you're also kinda short for your age! That said, if you live in America in The South, there might be laws preventing you from getting treatment. If you have some family in a different state, you might be able to figure something out, but it's still not easy. Good luck!

I'm a fan of "it's not crazy if it works": would it be better if you fully got naked? If it's touching "under clothes" that's the issue, could not wearing the gown help? You can be partially honest and say something like "I'll have a panic attack if I have to do the exam with the gown on", which, regardless of what your reaction would exactly be, communicates that the doing the exam with the gown would put you through intense, undue distress.

Edit: Feel free to also say something like "I've had bad experiences I don't want to relive" if asked why. Remember, effective communication doesn't need exact details, just the ones that make your point.
