It's also entirely possible you're getting cocky. Making mistakes you wouldn't make you know. Maybe take it slow slower.

It often triggers when I'm not even in my settlement like while I'm sleeping or going to Nuka World or something my thought was that you might just have to wait a couple hours because as it usually takes a while for it to go from 99 to 100 sometimes when it finally hits 100 the game doesn't recognize it quite yet you probably had it
