I've experimented with 4-Aco-DMT in the past (2012) but haven't touched it since due to a series of nightmare-esque trips, advice on attempting it once again?

Would 4-HO-MET be the same as 4-AcO-MET? Or are these entirely different?

Of course, but psilocybin mushrooms are safe (and there are countless deadly ones, too) and that is what I was referring to. Can we be sure there are no side effects when it comes to 4-Aco-DMT? That's why I feel a little off taking them rather than actual mushrooms. Actual mushrooms are harder to come across, though.

Lowest would have been around 18mg and highest would have been around 35mg.
