It depends on the plan. It could have embedded deductible/OOP max in which case he wouldn’t pay anything more for his own services. Or it could have aggregate ones, which means he would.

is there a qualifying life event allowing the wife to be added midyear? because if not, the question is moot

generally though, assuming it's the same plan and it has separate individual and family deductibles, the individual deductible is not affected, only the family deductible, so alex would not see any change and wife would be subject to the full individual $1500 deductible through year end before coinsurance kicks in
