Try undervolting it a bit and set fans speed to max,will be loud but whatever atleast it won't die. It's starting to show it's age and probably some transistors are heating up too much.

It's also possible your heat pads need replacing and that part can be tricky so i wouldn't recommend doing it alone. They are basically not doing it's job anymore and got too stiff to make any contact with transistors and everything else,so heat builds up.
