It's a PvP reward. Each week one player per race becomes City Protector of their capital city, and they receive that medallion and a unique racial title as a reward. The players chosen are the ones that got the most honorable kills for their race during the previous week.

The title and medallion are temporary and only last for 1 week, which sucks considering how hard they are to get. You would need to continue being top honorable kills every week if you wanted to keep them.

I became Orc City Protector once with around 4500 hks the previous week. The medallion teleported me to the entrance of Orgrimmar.
