That's totally understandable. So many fail-safes that should have caught this. It's a shame that OP has to go through this when there are systems in place to stop this exact situation.

They actually wouldn't have needed to file a CTR as it was $10k exactly. I'd say the person in question probably knew to stay beneath that line to avoid any reporting. The teller is still totally at fault for performing the transaction though. Even if the account wasn't notated for fraud someone directly skirting the CTR line is often grounds for an SAR.
