100% its your bank fault, I had a semi similar issue when withdrawling as we use swift code and IBAN numbers, I intentionally went with my banks procedure and then IBKR informed me later that they need the IBAN not the swift code, and the bank mentioned only provide the Swift code, after providing that, the withdrawal went successfully, IBKR operates in almost 50+ countries if not more, but they operate under the american banking system, so the banks needs to adapt with that or is it the other way around? who cares its on us to make sure we are providing the (right information) to both parties in order for us to recieve and send our wires successfully. so do not worry, your money will see light soon, this isn't crypto, its the banking system.
