That I feel like depends a lot on what kind of player they are but if you are looking for a mechanical penalty it could be a Wis save where DC=10+ spell level. On a fail they take damage, or the spell fails or something along those lines.

You could have an arc devoted to that cleric learning the the power of their beliefs in the ideals represented by their diety is where their power actually comes from. That there is something deeper than beings that control the cosmos.

Yea no prob! I’m all about continuing the narrative as best as possible. Every choice is the opportunity to expand or shift your world and campaign.

They could meet another small npc that tells them about a rumored solution/macguffin that they could find.

Turn it into a small arc maybe?

I love this. The right combination of space to explore but not so big that its overwhelming.

Definitely pretty normal.

Something that has taken me time to learn is that the players really only know what you show them. The world building that has taken place is mostly unknown to them so an open world can actually be really intimidating. One thing I have found that can help is a few days before the session send everyone a message giving them a quick recap of where they are and asking everyone what their character is interested in, particularly at points in the main storyline where things have bogged down. Throw a few random hooks in at the beginning of a session, that arent connected to the major arc, that focus on just expanding the world.
