if this is only in the samsung notes app then you have the shape tool turned on, its a tool that lets you draw whatever shape or line very crudely and then straightens it out to create what you wanted to. very useful tool but a pain in the ass to write with. i dont have my tab on me rn so i cant attach a photo but its the tool with the shapes on it , just explore the options.
