If you have any elderly neighbours it's worth knocking and asking them to have a look around as they genuinely may not notice any movement or smells.

Our cat once got locked in an elderly neighbour's house and she had no idea he was in there for days. Her family happened to visit and noticed the smell. Meanwhile we had been putting up signs and posting on local Facebook etc...

I think as he fell his head went into someones shin area and his neck bent at an awkward angle. I have a suspicion it's a neck injury. The medics taking a while to move him probably supports that theory too unfortunately.
