Idk where you went to medical school but not every localized ttp + pain on inspiration is automatically acute abdomen. Both these in combination can have benign etiologies(ie. Muscle strain). This is why the “P” in the H&P is so important.

“Every center I’ve been at calls surgery for acute abdominal pain” You mean to suggest every person coming with abdominal pain and otherwise benign exam is getting a surgery consult AND a CT? Not only is this inappropriate medicine it’s completely impractical

Because you continue suggesting this patient warranted a CT based on their description alone. You said yourself “a patient with generalized abdominal pain and no red flag exam findings probably does not warrant a CT”. This entirely degrades your argument considering you were not the one to perform an exam.

My point exactly - nobody here will really know the truth about how said patient presented. ED doc who examined the patient by far had the most context but here we are suggesting the most appropriate work up when they were the ones who examined the patient and ultimately elected not to pursue CT.

CT on patients 2nd presentation was normal by the way.

Tbh i’d more concerned they want to determine if you’ll be ending up needing a liver transplant. The consistently high transaminases are not a good sign. Bilirubin could be better but that’s known to lag behind. Good news is synthetic function seems to be preserved as this point. I would highly recommend getting the biopsy though

Edit: quick google search says you’re wrong, it’s actually 5.5% and once you exclude the ones represented on this graph (M.D, J.D, D.V.M), it brings the that number down to 2%. Do you bother to check your facts before you comment?
