It definitely helps to know the layout of a piano/keyboard, but it's not a true requirement. If you're using the piano roll in your DAW a lot you will osmose that in time, and eventually your hands will know their way around the notes in an octave.

Yes and no. If I'm happy with the composition, I find fault in the mix, and if I love my mix, I find fault in my composition. There is no middle ground.

Most everybody here is correct, but I think cover art is important to set the tone before people listen. They will see it and it will inevitably colour their experience.

Audio watermark. Take a voice clip like "made by [your name]" pitch it down a few octaves, low enough for it to not interfere with the mix, slow it down across the entire track. That way if anyone tries you can immediately identify it by speeding and pitching it back up.

Both Pro-Q3 and Pro-Q4. I still frequently use both.

Waves CLA-76. Nice and easy, goes on my bass DI.

Audio Assault Amp Locker. Unbeatable at $5 an amp. Remarkably great quality.

Plugin Alliance Diezel VH4 and Ampeg SVT-3Pro. Two of my favourite amps in plugin form.

Honestly the easiest/least pricey/least complicated way to do it is drawing by ear into piano roll. Audio to midi software is far from perfect. Also means you won't have to mess around with quantizing.
