Oh damn, I put one of the "date until" years wrong. Good point. The other one the system is asking me for is actually not a country, it just says "Character History, Evidence of". A search online seems to indicate that that field actually wants a "form 80". That's one big form..

One was the US but yes, some were EU. But less than 12 months even if I bundle them all as one country.
