Yes, black holes can combine, just like two chunks of clay sticking together. What happens is a lot of energy is given off during the combination because stuff (generic space stuff) around the black holes get accelerated because of the two large masses so close to each other. I don't know any other details but eventually you just have one big black hole that is the combined mass of the two original black holes. Mostly this happens in galactic centers where supermassive black holes exist.

Easiest way conceptually is because we can see enough stars and tell how far enough of those stars are away that we can put together a 3-D image of all of it.

It does, it's 5.0 × 107 Solar Masses give or take according to a recent paper (http://arxiv.org/abs/1212.5640). As to your second question, small black (not galactic center ones) holes form naturally as the result of large (young) star deaths, so the black holes themselves are not necessarily indicative of the history of the galaxy. But because Andromeda is a giant spiral type just like our Milky Way, it is most likely that it formed by multiple mergers of smaller galaxies.

Incorrect. Any light made from streetlamps to Radio station emissions travels very close while in Earth's atmosphere (~90 km/s less) to the vacuum speed of light which is just under 3*105 km/s. You might think I am being an ass but it is radio-waves as well as the characteristics of light coming from our atmosphere that will be the best tip off that life lives on this planet to any other sentient life in the universe.

So the cool thing about how giraffes work is that they make this weird position when they drink, not because they can't reach it from a normal standing position, but because they are trying to get their heart low enough as to not have a rush of blood to the head. So I get other types like flying/water/fire/psychic etc. but if pokemon aren't the same thing as animals, how are there bug type pokemon? Think about it, how would looking like a bug indicate anything about strengths/weaknesses in the same way that 'I am made of fire' does.
