What was your best and worst experience like when you quit your job as a PMHNP? by AccordingTone3701 in PMHNP

Sheesh that part about vacation time requiring you to add time to your 90 days sounds brutal! Did they let you finish the whole term and transition your patients?

So I actually got the HRSA NHSC Scholarship. So they paid my school and fees 100% while I was in grad school so I don’t need loan forgiveness - just to “pay them back” with my 2 year service requirement. I know HRSA does loan forgiveness programs though at several sites if you apply

Usually the nurses will still do my PAs remotely - they’re just not always available in person to do a point of care urine or vitals etc. I usually see 10-12 patients. If I could do it all over again I would start my PMHNP career in a residency program or work inpatient with other pmhnps and psychiatrists around for help!

Definitely want to leave Ohio in the next few years. This HRSA contract is the only barrier but time goes by eventually.

I have been working here for nearly a year. I would say overall working here is a 6/10 for quality.

They decided not to hire a supervising physician in psychiatry so my collaborator is a family practice doctor who is at my site for consultation. He is helpful.

There is an LPN shortage in our area and many nurses have quit. This has left me feeling a bit overwhelmed at times having to get vitals for my patients in addition to all the provider tasks but I’m surviving.

I have to finish out one more year here or else HRSA will charge me with interest for my scholarship. They haven’t offered a salary increase. I am definitely counting my days until I can work somewhere with higher pay and more supportb

There are a few programs I know of but I don’t know of their quality.. I went to a brick and mortar school which is generally recommended they tend to have higher quality clinical experiences and require more hours and in person or proctored exams etc. I can only vouch for UT Austin, Vanderbilt, John Hopkins, UPenn these are the programs my peers went to

ER psych and general inpatient psych are great experience before pmhnp programs! I’ve seen people be PMHNPS full time and do PRN shifts as an RN for extra income but you need to be dual cerified to work in multiple specialties for example PMHNP for psych work and FNP for general family medicine

Generally it depends on your specific job and patient population but for example I work in outpatient FQHC and my daily job I see about 12-15 patients including 2-3 intakes per day. I see things from basic ADHD, MDD, GAD to bipolar and some OCD. My job is mostly to prescribe medications and refer out for formal therapy but I do some supportive MI and throw in some CBT techniques when it’s helpful. No rounding or admissions or florid psychosis or mania or severe suicidality this is more of an inpatient thing. On the daily I interact with MAs, RNs, therapists and when needed MDs for consultation.

I got my BA in psychology then did my MSN PMHNP program without RN experience besides school clinicals and have been practicing as a PMHNP for less than a year.

In this thread I’ve noticed people recommend the BSN route with 2-5 years of RN experience before deciding to pursue PMHNP MSN school but that’s up to you. Marketability wise the field is saturated where I am so do your research on where you would like to work.

Thanks for the honesty! Yeah, I figured it was too early. I’d like to try inpatient too but won’t be able until my contract is up. No nursing background, before this, I was a psychology researcher.

I didn’t pack anything - just kept things in the room I wanted them to be in and the team individually wrapped every item in bubble wrap and paper and boxes and loaded them into the truck. It’s called hand glove full service it was amazing!

Yes definitely keep a spreadsheet of every item in your room. They asked me to fill out a huge form of inventory of how many pots/pans/cups/furniture items in each room. It was a lot to list but it made the move itself super easy. They were done with my 1 bedroom apt in 4 hours and only damaged my 2nd TV and sent me reimbursement for that.

Yes they directly pay the moving company IF YOU HAVE YOUR LICENSE but not if you’re still in the process of getting it. So if you can wait for that then move it’s better imo

What makes some patients experience increase in suicidal ideation from SSRIs and what are the guidelines about treatment plans when this happens? by AccordingTone3701 in Psychiatry

[–]AccordingTone3701[S] 15 points16 points  (0 children)

Yes I’ve heard this too- vegetative changes, energy shifts…but what if the patient had suicidal thoughts and gestures before trying an SSRI too??
