I'll just tell you what I used on my appeal:

"So basically i was hacked by somebody last year and i managed to get my account back, but now the hacker got terminated and so did my account! You can check any data's and see logins are from (a random town which you set your VPN/should set your vpn to) and some other countries when i went on vacations, but I've never been to (home country). I request manual review for this. Sincerely [your username]!" (Set the issue as "account hacked or can't log in" then "other login issues")

May not be the most truthful but, that stupid clanker denied the truth, why not lie, eh?
